I wouldnt say the game was scary, rather creepy. Sure there are times where they throw in cheap scare tactics (example: I rounded a corner, facing some morgue style metal freezer cabinets, and as I got closer, a guy in a butcher smock and a cleaver jumps out from a door and into my face and starts going at it), but mostly it's the way the environment is used to create a very great sense of unease. Like, The splicers themselves aren't terribly scary, but coupled with the fact that they speak eloquently in a gritty voice, are after you for different reasons than regular zombies, and the ambient music is playing 20's ragtime, it's quite an atmosphere.
